Rough Idle and Misfire: Causes in Order
Idle is where the engine can't hide
Under load, momentum and multiple cylinders cover for a weak one. At idle, every cylinder's contribution shows — which makes rough idle one of the most diagnosable complaints a diesel has, if it's tested instead of guessed at.
The usual causes, in rough order
- Injector problems — the leading cause: one or more delivering weakly or unevenly; often paired with white smoke at idle or a rhythmic miss
- Air in the fuel — suction-side leaks let air in, causing stumble, especially after sitting; often paired with hard starts
- Fuel supply and filters — marginal supply shows first at idle and low load
- Compression differences — a tired cylinder idles rough forever; valve lash on engines due for adjustment contributes
- Sensor/control faults — misreporting sensors make good hardware behave badly
Testing beats parts-swapping
- Scan for codes and look at cylinder contribution/balance data — modern ECMs will often name the weak cylinder outright
- Cut-out testing to isolate the miss
- Check for air intrusion and fuel supply health
- Compression/leakdown when the evidence points mechanical — and valve adjustment where it's due

It smooths out when warm — still a problem?
Cold-worse idle points at injectors, air intrusion, or compression — warming masks, not fixes. It's still worth diagnosing before it stops smoothing out.
Could bad fuel cause this?
Yes — contaminated or gelled fuel idles terribly. If it started right after a fill-up, say so when you call; it changes the first test.
